Dry Eye

Dry eye can cause daily discomfort, from gritty eyes to burning and blurry vision. Find relief with effective treatments, including Intense Pulsed Light (IPL) therapy.

What Is Dry Eye?

Dry eye disease occurs when your eyes are not able to maintain a healthy layer of tears on the surface.

This can happen for two main reasons. In many cases, either the glands around your eyes are not producing enough tears, or the tears you do produce evaporate too quickly because they lack the right balance of oil, water, and mucus.

That oil layer comes from tiny glands along your eyelids called the meibomian glands. When these glands become blocked or inflamed, a condition known as meibomian gland dysfunction (MGD), your tears break down before they can do their job. This is actually the leading cause of dry eye in most patients, more common than simple lack of tear production.

Left untreated, dry eye can also lead to ongoing inflammation on the surface of the eye, which tends to make symptoms worse over time and can affect the health of the cornea.

What Causes Dry Eye?

Dry eye can develop for many reasons, and often more than one factor is at play.

Common causes include:

  • Aging: Tear production naturally slows down as we get older, which is why dry eye becomes more common after age 50.
  • Meibomian Gland Dysfunction: Blocked or underperforming oil glands in the eyelids are one of the most frequent root causes of chronic dry eye.
  • Hormonal Changes: Menopause and other hormonal shifts can reduce tear production and change the quality of the tear film.
  • Autoimmune Disease: Conditions such as Sjogren’s syndrome, rheumatoid arthritis, and lupus are strongly linked to chronic dry eye.
  • Medications: Antihistamines, decongestants, antidepressants, blood pressure medications, and some birth control can all reduce tear production as a side effect.
  • Environment and Lifestyle: Dry or windy climates, indoor heating and air conditioning, long hours looking at screens, and extended contact lens wear can all contribute to symptoms.

How Is Dry Eye Treated?

Treatment for dry eye typically starts with the basics and moves toward more targeted therapies if symptoms persist.

Artificial Tears

Over the counter lubricating drops can offer short-term relief for mild cases.

Environmental and Lifestyle Changes

Adding moisture to the air at home or in the office can reduce tear evaporation, especially during Chicago winters when indoor heat runs constantly. Warm compresses can also help improve symptoms for some people. This gentle heat on the eyelids can help loosen blocked oil glands.

Prescription Drops or Ointments

For more persistent inflammation, medicated drops may be recommended to help the eyes produce healthier tears.

Punctal Plugs or Surgery

In more severe cases, small plugs, or in rare instances a procedure to close the tear drainage ducts, can help keep natural tears on the eye’s surface longer.

Intense Pulsed Light (IPL) Therapy

IPL was originally developed as a dermatology treatment, but doctors noticed that patients receiving it for skin conditions also saw an improvement in their dry eye symptoms. It is now one of the most effective in-office treatments available for dry eye caused by meibomian gland dysfunction, which is the case for a large majority of chronic dry eye sufferers.

How it works: During treatment, protective shields are placed over your eyes, and a cooling gel is applied to the skin around them. A handheld device then delivers gentle, filtered pulses of light to the area beneath the eyes and along the upper cheeks. That light energy is absorbed by small blood vessels near the eyelids and converted into a mild, controlled heat.

This heat does two important things. First, it helps soften and melt the thickened, hardened oils that are clogging the meibomian glands, allowing them to flow more freely into the tear film again. Second, it targets the tiny abnormal blood vessels that fuel ongoing inflammation around the glands, which helps calm the chronic irritation that keeps the cycle of dry eye going.

What to expect: IPL is a quick, in-office treatment with no drops or downtime. Most patients describe the sensation as warmth rather than pain, and you can return to your normal routine immediately afterward. A typical treatment plan involves a short series of sessions spaced several weeks apart, since gland function tends to improve gradually with repeated treatments.

Why patients choose it: Unlike drops or compresses, which mainly ease symptoms while they are being used, IPL is designed to improve the underlying function of the meibomian glands themselves. For many patients who have tried artificial tears and other over-the-counter options without lasting success, IPL can help target the source of the problem rather than just masking it.
